Medical Benefits

Employees have a choice between a High Deductible Health Plan (HDHP), which includes a Health Savings Account (HSA), or an Exclusive Provider Organization Plan (EPO). Please note per IRS regulation - employees covered by another medical plan may not enroll in the HDHP plan. Links to rates for medical premiums and information on HSA accounts are provided below.

Medical Plan Network of Providers are assigned and based on where you live. The County offers two Medical Plan Networks of Providers, Health Partners of Northern California (HPNC) and United Health Care (UHC).

Members who live in the local service area are assigned to HPNC. Members who live outside of the local service area are assigned to UHC. Please review the Local Service Area Zip Code List to confirm to which Medical Plan Network of Providers you will be assigned to.

If you reside in the Local Service Area Zip Code but have a dependent residing outside the Local Service Area Zip Code, the out of area provider network for HPNC members is First Health Complimentary. For information on how to access the online directory view the out of area provider directory.

Prescription Pharmacy Network

Amwell Telemedicine

The County’s telemedicine program provides convenient access, 24 hours a day, seven days a week, to a licensed physician using a smart phone, a tablet or a standard telephone. Telemedicine physicians can treat a variety of less-complex conditions such as colds, flu, allergies and infections as well as prescribe medications for the treatment of these conditions (limitations apply). Additional services include behavioral health (therapy and psychiatry), nutritional counseling and smoking cessation.

Health Bridges of Northern California

Health Bridges of Northern California’s primary mission is to provide health plan members a health BRIDGE to better health. Staff assists members navigate the health care maze with compassion and clarity so they can return to a place of better health. They are ready to assist you with: chronic care management services, hospital post discharge care management and nurse care managers to answer questions. Health Bridges also provides member liaison services to secure member appointments with in-network providers, assist with finding specialty providers and obtain specialty referrals, prior authorizations and arranging second opinion or transfer to a level 1 facility.
