Open Enrollment 2010

Each year the County holds open enrollment to allow employees, eligible for group health insurance, the opportunity to change insurance options and/or add previously uninsured eligible dependents. This year's open enrollment period is from November 6, 2009 through November 20, 2009 . All benefit changes made during this period will become effective January 1, 2010. Your department's Receiver must have all documentation for your selections returned to them no later than November 20, 2009 . Every employee must sign and return a 2010 County Benefit Enrollment Form this year . This will certify that every County employee was notified of the new rates, medical plan offerings and the flexible credit change. If you are currently enrolled in PacifiCare or HPSJ, you must elect a new medical plan.

Medical Insurance

The County is offering brand new medical plan options for you this year. Kaiser HMO is still being offered. However, PacifiCare HMO/POS and Health Plan of San Joaquin-Network Plus has been eliminated. The County is offering a new HMO medical plan through Anthem Blue Cross. Also, this year we are offering a brand new High Deductible Health plan through Kaiser and Anthem Blue Cross. These two HDHP's will be accompanied by a Health Savings Account. Please refer to the attached Benefit Comparisons for plan descriptions. For more information, please attend a Open Enrollment meeting and/or HSA presentation.

The HMO medical plan will still remain funded at the lowest cost HMO option at the coverage level of choice. The High Deductible Plan will also be funded at the lowest cost HDHP option at the coverage level of choice.

Health Savings Accounts

If you elect to enroll in a High Deductible Health Plan, you will automatically be enrolled in a Health Savings Account (HSA). The HSA account will be funded with the deductible based on your coverage level option. For calendar year 2010, Employee only deductible will be $1,250, EE +1 and Family deductible will be $2,500. The HSA account will be funded on the first paycheck in January with half of the deductible amount, $625 Employee only and $1,250 Family. The remaining balance will be funded semi-monthly beginning in July 2010.

You may also elect to voluntarily contribute to the HSA account up to a maximum amount based on coverage level. Employee only maximum for 2010 is $3,050, Employee + 1 and Family maximum is $6,150. The funds in this account may be used for qualified medical expenses. This account is similar to the Flexible Spending Health Account (FSA). The difference is, your money stays with you forever.

Note- You may not be enrolled in both the HSA and the Flexible Spending Health Account .

 

Dental & Vision

The County will provide 100% paid Dental and Vision at the coverage level of your choice. However, the Flexible Benefit Credit to purchase Dental and Vision will be eliminated; therefore if you had a Flexible Excess left over last year, you will no longer receive this excess cash in your paycheck. If you are currently Waiving Dental and Vision coverage, you will no longer receive the flexible credit for waiving, therefore, if you would like to enroll in the county paid Dental and Vision, you will need to complete the attached Universal Enrollment Form and submit with your Benefit Enrollment.

The County has decided to change the Dental Plan vendor from Stanislaus Dental Foundation to Delta Dental PPO plus Premier. This is a vendor change only. The Dental plan coverage has not changed. Your percentage level will not change. We plan for this to be a seamless transition.

 

Supplemental Life Insurance

Supplemental Life and AD&D is a total voluntary election.  Therefore, the County does not provide a credit towards the premium.  There is also an Administrative fee built into the rate.

** If there is a new enrollment or increase made to Supplemental Life insurance, an Evidence of Insurability Form must be completed and attached to the Benefit Enrollment Form. This form can be obtained from your department payroll/personnel clerk or the Employee Benefits Website. The increased amount will not be reflected until this form has been completed and approved by the underwriter.

 

Flexible Spending Accounts- Health and Dependent Care

The Flexible Spending Health and Dependent Care Account enrollment expires at the end of every plan year. Therefore, you must re-enroll in these plans for the new 2010 plan year. If we do not receive your enrollment form to re-enroll during the 2010 open enrollment period, you must wait until the following year's open enrollment period.  No exceptions.

The contribution for these plans is semi-monthly (24 pay-periods). To enroll, you must complete page 3 of the County Benefit Enrollment Form. This year County employees may elect to sign up for a free WageWorks Debit card to be used with your Flexible Spending Account. Note- If you enrolled in a High Deductible Health Plan with an HSA, you are not eligible for the FSA Health Account.
