The following blank forms are intended to be printed first, then completed. Please note that you cannot submit any of these forms on-line. If you have problems or concerns about printing the forms from your computer, please contact our office and we can mail the forms to you. - Fictitious Business Name Statement Application
- Preliminary Change of Ownership Report (PCOR)
Required for any document that changes formal title to real property. The party receiving interest in the property must sign the PCOR. If the required PCOR is not submitted or not signed by the correct party, an additional $20.00 fee will be due. Recording fees are not charged for the PCOR.

Required when there is not sufficient space for the recording information on the first page of a document. Recording fees are charged for this page.
- Marriage License Application
Both bride and groom must be present to apply for a marriage license.
