Spanish Language Capabilities
A Mental Health Services Act funded multi-disciplinary program that provides intensive, integrated services to individuals 18 years and older who have both a serious mental illness as well as diabetes or hypertension. Services include case management, counseling, family education, medication and rehabilitation services. Support is available to clients on a 24/7 basis. The team works closely with family physicians. In addition, individuals are linked to community support groups and encouraged to develop their own peer support and recovery groups.

Spanish Language Capabilities
A Mental Health Services Act multi-disciplinary program providing comprehensive mental health and co-occurring services for adults (18 years and older) on probation who are underserved and either homeless or at risk of homelessness, have frequent contact with law enforcement, and/or are frequent users of hospital and emergency room services. Services are also available to qualifying individuals coming out of state hospitals and some Drug Court participants with a co-occurring disorder. Services include: case management, crisis response, family support, housing and employment assistance, mental health rehabilitation, medication support, peer support and 24/7 coverage.
A Mental Health Court will be available later in 2007 as a result of a partnership with the Criminal Justice System. This service will provide treatment which has shown to reduce the amount of time an individual spends in jail and can reduce the recidivism rate.
